With a title-page woodcut. Printer's name from STC. Signatures: A-D⁴ E² . Quire A is partly reimposed from STC 5654.5. Identified as STC 5655 on reel 1443. Reproductions of the originals in the Henry E. Huntington Library and Art Gallery (reel 526) and the British Library (reel 1443).
