Publisher's name conjectured by STC. The text proper begins new pagination and register, with caption title: A refutation of M. Whitakers reprehension .. Includes index and a final errata leaf. Quire 2P occurs in two settings; 2P1r line 11 is (1) "See Heauen." or (2) "107. See Heauen.". Reproduction of the original in the Henry E. Huntington Library and Art Gallery.
